POSITION DESCRIPTION:
This position will be assigned to the Regional Office Region 4 Front Door Team. As such will assist individuals and families seeking and/or amending OPWDD services from all 5 boroughs.
Primary Duties and Responsibilities
Staff assigned to the Front Door division at the Regional Offices serve in a variety of capacities including, but not limited to:
Service Amendment (SARF) o Will be regularly assigned Service Amendments across 5 boroughs for review and service authorization decisions o Interface with other service teams (ie.:
self-direction, front door) as needed to troubleshoot and resolve decisions. o Will interact with care managers regularly as part of the review process. o Will participate in weekly region wide Quality Review meetings o Will utilize One Drive; Excel; Outlook; TABS & CHOICES daily to complete tasks o Will complete the NOD.09 to make service decisions. Local School Transition (LST) o Will serve as the liaison to the families whose children are about to graduate from high school and are seeking to obtain services. o Will assist with conducting information sessions at the local schools, sending out eligibility documents, conduct assessments, complete pre-authorizations, and consult with Care Coordination Organizations. o Will interface with hundreds of families who have individuals that are about to graduate from high school, or who have already graduated from high school. o Will prepare quarterly reports and coordinate multiple meetings with organizations, schools and DDRO staff. o Will continue to work with those families as they progress through the authorization process as well as provide ongoing assistance to the families who have not yet completed the Front Door process. Salary Range:
From $57984 to $73813 Annually
Minimum Qualification
o Must currently be permanent in the title of Social Worker Assistant 3, OR o Must be active and reachable on the appropriate Civil Service eligible list, OR o Must be eligible for 70.1 transfer by way of current permanent status in a title deemed comparable by the NYS Department of Civil Service. FOR PROVISIONAL APPOINTMENT:
o Must possess a Master's degree in Social Work from a regionally accredited college or university, OR A Bachelor's degree in Social Work and one year of post bachelor's experience in Social Work, OR A Bachelor's degree or higher in a human service area and two years of post-bachelor's experience in Social Work. If you are appointed provisionally, you must take the next scheduled civil service examination for this title and your score must be reachable on the resulting eligible list before you can be permanently appointed. If you do not pass the examination or achieve a reachable score, you may not be retained Estimated Salary: $20 to $28 per hour based on qualifications.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
