Case Manager

An esteemed community center dedicated to providing expert social services to individuals and families, spanning children, youth, and adults is currently seeking Case Managers for its Manhattan location. As the Case Manager you'll be providing impactful case management services utilizing a strengths-based, client-centered approach. This individual will collaborate with the social service team to deliver case management services to formerly homeless adults diagnosed with severe persistent mental illness, supporting them in maintaining permanent housing.
The base pay range for this position is $22 to $24 hr. The actual compensation offered to the successful candidate may vary from the posted hiring range based on geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of the offer. Responsibilities:
Offer case management services to a designated client group, pinpointing both short and long-term treatment objectives and assisting clients in reaching them. This involves connecting clients with community programs like medical, psychiatric, and substance abuse treatment services.
Additionally, coordinate and monitor interactions with medical and psychiatric treatment providers.
Attend weekly staff meetings, case conferences, training sessions, and supervision meetings.
Work together to keep an updated resource directory.
Perform any additional responsibilities as necessary in alignment with agency policies and funding source regulations, such as incident reporting, data gathering, and preparing for audits.

Qualifications:
Bachelors Degree
Prior experience working with individuals who experience mental illness and substance abuse disorders

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
