What You'll Do:
In alignment with the New York State NYS Medicaid State Plan Amendment SPA for Health Homes HH , The Mount Sinai Health System MSHS is committed to referring eligible individuals who seek treatment in hospital emergency departments ED to designated HH Providers within Mount Sinai Health Partners MSHP and the Mount Sinai Performing Provider System MS PPS .
MSHP is the management services organization developed to enhance the Health System's implementation of population health initiatives, that is, programs of care that enhance the patient experience and health status, improve the health of patient populations and reduce utilization and healthcare costs.
The MS PPS is the largest of the 11 PPSs in NYC, all serving as part of the Delivery System Reform Incentive Payment (DSRIP) Program, a NYS Department of Health NYSDOH initiative, which aims to redesign NYS's health care delivery system for all Medicaid beneficiaries to increase quality of service delivery and reduce costs (reduction of ED visits and hospitalizations by 25% across NYS is the ultimate utilization goal) over its five years, ending in March 2020. MS PPS partners with 100 community management agencies CMAs and six lead HHs in Manhattan, Brooklyn and Queens to complete this work.
This NYSDOH Referral to HH Initiative aims to connect eligible patients to HHs which offer the opportunity and resources to provide coordinated care that can improve the health and well-being of patients. HHs provide comprehensive care management for NYS Medicaid Beneficiaries with two or more chronic conditions, HIV/AIDS, or serious mental illness (SMI). HH care managers work with the HH's network of providers, health plans, and community-based providers to facilitate comprehensive, person-centered care planning.
The Program Manager is responsible for the direction setting, continuous improvement and maintenance of the current NYS SPA for HHs, focused on MSHS hospital ED referrals, providing administrative leadership. The Program Manager will lead the strategy around the future NYS SPA for HHs, focused on MSHS hospital inpatient referrals. The Program Manager will oversee IT systems development, compliance with NYSDOH regulations and audits, workflows for in-person and telephone HH notification and outreach, data collection and analysis and performance improvement in all of these areas. The Program Manager will lead the NYSDOH Referral to HH Initiative Steering Committee meetings, comprised of key leadership stakeholders across MSHS and report out on all aspects of the mandate identifying areas for performance improvement and the advantages/disadvantages of solutions for consideration.
What You'll Bring:
RESPONSIBILITIES
Oversees daily operations of NYSDOH Referral to HH initiative at all implementation sites. Works with key internal (MSHS) and external (CMA) stakeholders to manage staffing, workflows, IT support, data tracking and reporting and provides regular performance feedback, in accordance with policy & procedure. Models and inculcates organizational values.
Represents MSHS on this mandate with NYSDOH and remains current on all regulatory aspects by attending NYSDOH meetings and webinars, reading NYSDOH related documentation. Keeps key stakeholders across MSHS informed of important aspects, identifies areas of vulnerability and potential ideas for compliance and overall performance improvement.
Resolves issues and improves services and outcomes by maintaining cooperative working relationships with leadership in the following areas: MSHS ED, onsite CMA(s), MSHP care management, Call Center, IT, Human Resources and MS PPS. This includes leading workgroup meetings with these service areas to work collaboratively on service improvement to meet desired outcome measures.
Serves as MSHS lead for regulatory audits by NYSDOH. Leads regular self-audits of performance and identifies areas for improvement and develops performance improvement plans with key stakeholders accordingly.
Develops policies and procedures for all aspects of NYSDOH Referral to HH Initiative to meet current goals and objectives and to anticipate future strategic changes.
Accepts and performs other duties as assigned.
REQUIREMENTS
Master's Degree preferred.
Demonstrated leadership experience in innovative programs with complex operational and regulatory requirements.
Exceptional organizational, collaborative, communication and presentation skills to meet the challenges of a complicated, evolving mandate.
Significant experience working in hospitals, Health Homes (leads or care management agencies), or community-based organization(s).
Experience working with government regulatory agencies, participating in audits and monitoring compliance.
Exceptional computer proficiency including data analysis skills to implement programmatic change.
Commitment to an underserved patient population.
Strong interest in being part of an innovative initiative to improve the quality and cost of care.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
