What You'll Do:
Mount Sinai Health Partners (MSHP) is the management services organization developed to enhance the Health System's implementation of population health initiatives, that is, programs of care that enhance the patient experience and health status, improve the health of patient populations and reduce utilization and healthcare costs. Population health initiatives particularly focus on preventive care.
MSHP provides the infrastructure to support population health initiatives and includes services such as care coordination, information technology, workflow optimization, physician engagement and quality reporting.
The Department of Social Work Services is a leader in the development of MSHP and provides transitional and longitudinal care coordination services for a large group of individuals covered by diverse insurances.
The role of the Social Work Program Coordinator for the MSHP Volunteer Partners Program is to expand this highly successful program across the Health System. The Volunteer Partners Coordinator will engage a large team of volunteers; provide ongoing training, supervision, and recognition to volunteers; enhance patient/family satisfaction; and continue to demonstrate the significance and potential of the volunteer role in reducing avoidable readmissions.
MSHP Volunteer Partners are a group of volunteers who work on the team with social workers, thereby increasing the numbers of patients MSHP can serve. These volunteers are in the MSHP offices for four- to eight-hour shifts, Monday-Friday, 9am-5pm.
What You'll Bring:
New York State LCSW.
Ability to manage a complex volunteer program working with MSHP staff across 6 sites.
Experience recruiting, training, supervising volunteers preferred.
Mount Sinai Health Partners experience preferred.
Exceptional skills in engaging patients and families and in assessment and coordination of resources within families and hospital and community networks for effective management of patient care.
Exceptional organizational, collaborative and psychosocial assessment and intervention skills.
Exceptional ability to engage, motivate, supervise a diverse group of volunteers and collaborate with MSHP social workers to use them effectively.
Strong interest in being part of an innovative initiative to improve the quality of care by addressing preventable readmissions.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
